Course and Offering(s) Registration Submission Attestation

When submitting a course and its offering(s) to ASHA Continuing Education (CE), the ASHA Approved CE Provider is attesting to the following:

Having a process for identifying and selecting qualified instructional personnel.
Posting its cancellation, refund and complaint policies for course registrants' review.
Providing instructions on requesting special accommodations to course registrants.
Having a process for evaluating the course and its offerings.
Ensuring that instructional personnel disclosure statements are announced at the start of each course offering.
Retaining information about the planning, implementation, evaluation and records of attendance for this course and all its subsequent offerings for 7 years after the end date of the last course offering.
Reviewing the course registration confirmation and notifying the CE Staff of any corrections within 15 days of receipt.
Reviewing the CE Course Roster and notifying the CE Staff of any corrections within 45 days.
Retaining the participants' names and information on ASHA CEUs earned for a minimum of 2 years from the completion date of each course offering.

For a course and/offering(s) filed beyond the Continuing Education Board's (CEB) initial filing deadlines: The Provider will work with their CE Provider Manager to discuss how they can better manage course planning to meet ASHA CE course registration deadlines.

For an additional offering(s) filed beyond the CEB's additional offering deadline: The Provider will contact their CE Provider Manager if an additional offering is beyond the 3-day filing deadline.

For Providers with a CE Content Consultant: The Provider ensured that the CE Content Consultant participated in the planning of the course and has reviewed the Course and Offering Registration information.

For Cooperative Course Offering(s): The Provider acknowledges that they were contacted in advance of the planning and promoting of this course. The Provider takes full responsibility for having worked closely with the cooperative party to ensure that all of the ASHA CEB requirements were met.
