Participant Appeal Form

Submit this form if the following applies to you:

  1. A course you took for ASHA CEUs is not showing up on your CE Registry Transcript AND
    1. You have contacted the Provider to make sure that
      • The course was offered for ASHA CEUs
      • They have a record that you requested ASHA CEUs
      • They didn’t make an error and did not report you. 
    2. You successfully completed the course (live or online) and all required tasks (quiz, evaluation form, etc.).
    3. It has been more than 8 weeks since you finished the course.
    4. It has been less than 12 months since you finished the course.
    5. You paid any fees that the Provider charges to be reported to ASHA CE.
  2. You missed earning an Award for Continuing Education (ACE) because the completion date shown on your transcript is not the actual date you completed the course
  3. You believe that the number of ASHA CEUs on your ASHA CE Registry Transcript for a course is not correct.

Please note: Appeals are not approved for courses you completed more than twelve months ago.

Submission of this Participant Appeal Form does not guarantee that ASHA CEUs will be awarded. To earn ASHA CEUs, you must pay the CE Registry fee for the year that the course occurred.
